A 38-year-old patient with AIDS presents
with increasing fatigue, malaise and weight loss of 10 pounds over 3 weeks.
He has no other symptoms and his physical examination is normal except for the
findings shown in the image on the left.
You note that his serum viral load was undetectable 6 months ago;
3,800 copies/ml 3 months ago; and 18,000 copies/ml currently. You decide
that his current antiviral regimen of zidovudine, lamivudine, and
stavudine is no longer efficacious, and you decide to change his drug regimen.
