Question:
A 20-year-old female has shortness of breath with minimal activity, weakness, chronic fatigue, and pedal edema. She first noticed she became short of breath rather easily about one year ago, but wasn't concerned because she had gained about 15 pounds. Since that time she has lost the weight, but the dyspnea has worsened. On several occasions she has almost "passed out." Her feet and ankles began to swell approximately two months ago.
On exam, the patient is alert and oriented. Vital signs are within the normal range. The carotid pulse is reduced and there is an easily palpable ventricular lift. An S3 and S4 are heard. The liver is palpable 2cm below the costal margin. What is her diagnosis?
- tricuspid stenosis
- primary pulmonary hypertension
- aortic stenosis
- mitral stenosis
