Early ECG Changes of Acute Pericarditis
In early acute pericarditis, there is diffuse epicardial injury with diffuse elevation of most ST segments, except for lead aVR, which overlies the valvular base of the ventricles. The ST segment and T waves vectors initially point in approximately the same direction. The PR (PQ) segments are often depressed in leads with upright P waves due to atrial injury.

RS: ECG Muscle Tremor or Motion Artifact
Note the rapid, irregular spikes caused by muscle tremor. These are due to electrical interference caused by depolarization of the patient's tremulous muscles. This can be remedied by getting the patient to relax, giving him a warm blanket if he is shivering, or moving the limb leads closer to the trunk (if limb tremor is responsible).
Findings summary:
> Sinus tachycardia
> Motion artifact
